fixed the issue. Will look into that further Set your column as 'set' (specify the all possible values.) data type and than run the below query.
$comma_separated = implode(",", $values);
$insert_query = "INSERT INTO TABLE_NAME(col_name) VALUES('$comma_separated')";
$result_insert = mysql_query($insert_query);

will help you You have not posted the table structure but anyways I will try to give my best suggestions here.
Existing System:
id,(other columns),choice1, choice2, choice3, choice4
checkbox table
--------------

id, checkbox_name, checkbox_value(if any)

teachers table
--------------

id, cols (other columns which I dont know whether its there or not)

teachers_choices //This is the checkbox selected table
----------------

id, teacher_id(this references to teacher table), checkbox_id (this references to checkbox table)

it fixes the issue When I store services and activity then services and activity value store multiple times in the database, I also use implode instead of foreach but that time first service and first activity stored, the following image describes the idea about what I exactly want.
